It is the cancer that is confined to the mucosa or submucosa regardless of the presence of lymph node metastases

when do we do subtotal and total gastrectomy ?

The selection of the surgical procedure in patients with gastric cancer should be primarily adjusted to the location of the tumor, the growth pattern seen on biopsy specimens, and the expected location of lymph node metastases.
Total gastrectomy is recommended if the biopsy shows a diffuse carcinoma.
For distal-third gastric cancer, patients may undergo subtotal gastrectomy if biopsy reveals intestinal type of adenocarcinoma
